Hi. i have just joined a new workplace and the working culture is very toxic. There is too much of negativity that i can sense in the new place. I am trying to avoid that but i am unable to. Due to this, i am most of the times depressed and as a result have started eating less and sleeping less. I do not have sound sleep and get panic attacks all day and night. i am not sure about the future and it seems all lost to me. I need to come out of the situation but do not know how to handle it. i tried meditation but it becomes very difficult once i close my eyes. do i need a therapist to come out of this situation?
Panic attacks (or anxiety attacks - the terms are interchangeable) are intense episodes of fear which are so powerful that they trick you into fearing that you are dying, going crazy, about to faint, or losing control of yourself in some vital way. The symptoms of a panic attack feel so powerful and threatening that they convince you that you're in terrible danger.
Because symptoms are so severe, many people who experience a panic attack may believe they are having a heart attack or other life-threatening illness and may go to a hospital ER. Panic attacks may be expected, such as a response to a feared object, or unexpected, apparently occurring for no reason. The mean age for onset of panic disorder is 22-23. Panic attacks may occur with other mental disorders such as depression or PTSD.
Mindfulness involves spending time focusing on the present moment and using a nonjudgmental stance (things are not good or bad, they just are). This may sound straightforward but it can be tricky as our mind often wanders. Try to spend some time each day focusing on a single activity for 10 minutes. For example, focus on the experience of breathing: noticing the physical sensations that you have, the sound of your breath, the feeling of your chest rising and falling as you breathe, the feeling of air entering and leaving your lungs, etc. Try your best to keep your mind focused on these sensations. If you notice your mind wandering, gently redirect it back to the exercise. Engaging in these exercises on a regular basis can help you to feel emotionally centered. Loved ones, as well as medical personnel, generally attempt to reassure the panic attack sufferer that he or she is not in great danger. However, these efforts at reassurance can sometimes add to the patient's struggles. If the doctors say things like, "it's nothing serious," "it's all in your head," or "nothing to worry about," this may give the false impression that there is no real problem, they should be able to overcome their symptoms without help, and that treatment is not possible or necessary. More accurately, while panic attacks can undoubtedly be serious, they are not organ-threatening. Therefore, for people who might wonder what to do to help the panic sufferer at the time of an anxiety attack, a more effective approach tends to be acknowledge their fear and the intensity of their symptoms while reassuring the person having the panic attack that what is occurring is not life-threatening and can be treated.
The psychotherapy component of treatment for panic disorder is at least as important as medication. Anxiety disorders differ from normal feelings of nervousness or anxiousness, and involve excessive fear or anxiety. Learn how to control your breathing. Hyperventilation brings on many sensations (such as lightheadedness and tightness of the chest) that occur during a panic attack. Deep breathing, on the other hand, can relieve the symptoms of panic. By learning to control your breathing, you can calm yourself down when you begin to feel anxious. And if you know how to control your breathing, you’re also less likely to create the very sensations that you’re afraid of.
But flashbacks may occur with other types of anxiety as well. Some research, including a 2006 study in the Journal of Anxiety Disorders, suggests that some people with social anxiety have PTSD-like flashbacks of experiences that might not seem obviously traumatic, such as being publicly ridiculed. I’m 15 years old and this is something very similar that happens to me everyday, it sneaks up on me at random times. It is a terrible feeling and almost uncontrollable. It started around 5 months ago when my grandfather passed away, I went to the the hospital atleast 5 times and I even get suicidal thoughts sometimes because the feeling is terrible and something I don’t wanna go through everyday. The prognosis for people who suffer a panic attack is overall, good. Some people have a single attack and are never bothered again. Yet, two-thirds of people experiencing a panic attack go on to be diagnosed with panic disorder. Also, half of those who go through a panic attack might develop clinical depression within the following year, if not treated promptly. A panic attack is a response of the sympathetic nervous system (SNS). The most common symptoms include trembling, dyspnea (shortness of breath), heart palpitations, chest pain (or chest tightness), hot flashes, cold flashes, burning sensations (particularly in the facial or neck area), sweating, nausea, dizziness (or slight vertigo), light-headedness, hyperventilation, paresthesias (tingling sensations), sensations of choking or smothering, difficulty moving, and derealization. These physical symptoms are interpreted with alarm in people prone to panic attacks. This results in increased anxiety and forms a positive feedback loop.[10]

Everyone has probably experienced panic, or something like it, at least once in their lifetime: on a disturbingly turbulent plane, or before giving an important presentation, or after realizing you hit reply all when you really, really should not have. We all know the paralyzed feeling and the heightened physical sensations. But panic attacks and panic disorder take a different shape. Panic attacks have many physical symptoms and tend to peak around 10 minutes, and may last for 30. Panic disorder is diagnosed by the frequency of these attacks, and the presence of a fear of having them. 


Until recently, panic disorder was not distinguished from agoraphobia (distressing anxiety resulting from being outside the home, travelling via public transit, being in open or claustrophobic environments, or being in crowds that generally leads to extreme avoidance due to fear of not being able to escape in those situations; APA, 2013). Current estimates contend that just under 2% of teens and adults have agoraphobia (Kessler et al., 2012). Often, people associate panic attacks or their panic disorder with certain places, people, or events and the fear of another attack occurring can lead to comorbid fears of the location or idea that there is no escape, which leads to extreme avoidance. Panic disorder can also occur simultaneously with other anxiety disorders, such as Generalized Anxiety Disorder (GAD) and/or Social Anxiety Disorder (SAD), for example.
I’m not sure if this counts as a panic attack, but lately I’ve experienced instances where my head feels like it’s being squeezed, I feel really dizzy, and I get an intense fear of becoming schizophrenic because In that moment it feels like I’m going crazy. It’s estimated that 19 percent of the North American adult population (ages 18 to 54) experiences an anxiety disorder, and 3 percent of the North American adult population experiences anxiety attack disorder. We believe that number is much higher, since many conditions go undiagnosed and unreported.
Many patients first report symptoms to their primary care physician. Primary care physicians (PCPs) will administer a thorough physical exam to rule out hormonal imbalances, side effects of medications, and certain illnesses. If the symptoms are not due to other conditions, the physician may diagnose the patient with anxiety and therefore refer the patient to a psychologist or psychiatrist. Physicians practice in hospitals, clinics and private practices.

However, no one theory has been determined so the underlying reason for sex differences in rates of panic disorder remains unknown.

You may be anxious because of an overly demanding schedule, lack of exercise or sleep, pressure at home or work, or even from too much caffeine. The bottom line is that if your lifestyle is unhealthy and stressful, you’re more likely to feel anxious—whether or not you actually have an anxiety disorder. These tips can help to lower anxiety and manage symptoms of an anxiety disorder:
Social risk factors for anxiety include a history of trauma (e.g., physical, sexual or emotional abuse or assault), early life experiences and parenting factors (e.g., rejection, lack of warmth, high hostility, harsh discipline, high parental negative affect, anxious childrearing, modelling of dysfunctional and drug-abusing behaviour, discouragement of emotions, poor socialization, poor attachment, and child abuse and neglect), cultural factors (e.g., stoic families/cultures, persecuted minorities including the disabled), and socioeconomics (e.g., uneducated, unemployed, impoverished although developed countries have higher rates of anxiety disorders than developing countries).[57][89]
The buildup phase of a panic attack is the most important phase, because it is when you can prevent it. If you are under chronic stress, you become worn down and have no reserve left. Anything can push you over the edge. But if you regularly reduce your stress through a combination of cognitive behavioral therapy and mind-body relaxation, you will have enough reserve left to handle life’s surprises.
A variety of medical and mental health professionals are qualified to assess and treat panic disorders. From purely medical professionals like primary care doctors, emergency room physicians to practitioners with mental health training like psychiatrists, psychologists, and social workers, a variety of health care providers may be involved in the care of panic disorder sufferers. Some practitioners will administer a self-test of screening questions to people whom they suspect may be suffering from panic disorder. In addition to looking for symptoms of repeated panic attacks using what is described in the Diagnostic and Statistical Manual of Mental Disorders (DSM-5), asking detailed questions about the sufferer's history and conducting a mental-status examination, mental health professionals will explore the possibility that the individual's symptoms are caused by another emotional illness instead of or in addition to the diagnosis of panic disorder. For example, people with an addiction often experience panic attacks, but those symptom characteristics generally only occur when the person is either intoxicated or withdrawing from the substance. Someone who has post-traumatic stress disorder (PTSD) may have panic attacks when reminded of trauma they experienced and in a person with obsessive-compulsive disorder, panic attacks may be triggered by their being unable to perform a compulsive behavior. The practitioner will also likely ensure that a physical examination and any other appropriate medical tests have been done recently to explore whether there is any medical problem that could be contributing to the occurrence of panic attacks. That is particularly important since many medical conditions may have panic attacks as a symptom and therefore require that the underlying medical condition be treated in order to alleviate the associated anxiety. Examples of that include the need for treatment with antibiotics for infections like Lyme disease or vitamin supplements to address certain forms of anemia.

The disorder in younger children is less likely to have the symptoms that involve ways of thinking (cognitive symptoms). Both children and teens with panic disorder are further at risk for developing substance abuse and depression as well as suicidal thoughts, plans, and/or actions.


The emotional effects of anxiety may include "feelings of apprehension or dread, trouble concentrating, feeling tense or jumpy, anticipating the worst, irritability, restlessness, watching (and waiting) for signs (and occurrences) of danger, and, feeling like your mind's gone blank"[20] as well as "nightmares/bad dreams, obsessions about sensations, déjà vu, a trapped-in-your-mind feeling, and feeling like everything is scary."[21]
In deeper level psychoanalytic approaches, in particular object relations theory, panic attacks are frequently associated with splitting (psychology), paranoid-schizoid and depressive positions, and paranoid anxiety. They are often found comorbid with borderline personality disorder and child sexual abuse. Paranoid anxiety may reach the level of a persecutory anxiety state.[53]
Because involuntary panic attacks can be caused by other medical conditions, such as mitral valve prolapse, thyroid problems, hyperglycemia, side effects from certain types of medications, recreational drug use (such as marijuana), stimulants, etc., it’s best to discuss your panic attacks and symptoms with your doctor to rule out any medical cause.

Yes. There are many medications that have FDA approval to treat anxiety disorders. Several members of the benzodiazepine class are routinely used to provide relief from anxiety. These minor tranquillizers are safe and effective, but should be used for short-term relief. They have many side effects, including drowsiness, and can be habit forming at higher doses. People taking these medications should not use heavy machinery or drive until they understand how the medication might affect them.
I just had my first big anxiety attack yesterday. It happened probably because of some friend drama and school responsibilities and how the world is completely crumbling down and I can’t do anything about it. I started sobbing uncontrollably, I was hyperventilating so oxygen couldn’t get to my hands so they started going numb I was alone for about 30 minutes until I had the strength to call my mom. A person with panic disorder experiences sudden and repeated panic attacks—episodes of intense fear and discomfort that reach a peak within a few minutes—during which time the individual experiences physical symptoms such as chest pain, heart palpitations, breathlessness, vertigo, or abdominal distress, sometimes accompanied by the fear of dying or of going insane.  These symptoms may seem similar to those of a heart attack or other life-threatening medical conditions. Panic disorder is often diagnosed after medical tests or emergency room visits have ruled out other serious illnesses.
The mutism must also include impairment in social, academic, or occupational achievement or functioning to qualify as a diagnosis. Selective mutism is not present if it is related to lack of knowledge or comfort with the spoken language required of the situation or is due to embarrassment from a communication or developmental disorder. Experiencing a chronic medical condition or severe or frequent illness can also increase risk for anxiety disorders, as well as dealing with significant illness of a family member or loved one. Given that several medical conditions have been linked to significant anxiety, in some cases a physician may perform medical tests to rule out an underlying medical condition. For instance, thyroid disease is often characterized by experiencing significant symptoms of anxiety. Menopause, heart disease, and diabetes have also been linked to anxiety symptoms. Additionally, drug abuse or withdrawal for many substances is characterized by acute anxiety, and chronic substance abuse can increase risk for developing an anxiety disorder. Anxiety can also be a side effect of certain medications. Experiencing significant sleep disturbances, such as difficulty falling asleep or staying asleep, may also be a risk factor for developing an anxiety disorder.
Many who have never dealt with this ailment dismiss it as an excuse by those who suffer from it to get out of mundane tasks or work requirements. Much like ADHD or mood disorders, anxiety is not something that can necessarily be seen or proven when you are the sufferer struggling to explain yourself to someone who doubts that your night sweats and inability to turn your brain off in the wee hours of the morning are not something you are making up to take a vacation day. Our life is not a vacation.
The last strategy — learning what triggers your anxiety — is important. Sometimes you can take small steps to conquer your anxiety instead of letting the trigger conquer you. For example, if meeting new people causes you high anxiety, consider going with a friend to meet the new neighbors. Once you do this with ease, you can move forward and meet people on your own. All the pent-up fear and anxiety attacks will start to resolve as you become accustomed to reaching out in your community.
The cognitive effects of anxiety may include thoughts about suspected dangers, such as fear of dying. "You may ... fear that the chest pains are a deadly heart attack or that the shooting pains in your head are the result of a tumor or an aneurysm. You feel an intense fear when you think of dying, or you may think of it more often than normal, or can't get it out of your mind."[22]
Panic attacks are often confusing for the sufferer. They are usually sudden and are accompanied by extremely intense physical sensations, leaving one to believe they may have a serious medical condition. Because the physical symptoms associated with a panic attack are similar to certain serious medical conditions, it is important to rule out any medical causes.
Anxiety is becoming increasingly prolific in today’s society, particularly among young people. While everybody feels anxious at some point in their lives, anxiety disorders can be all-encompassing unless you seek help. But what exactly is anxiety, and how do you treat it? The main type of anxiety is referred to by health specialists as generalised anxiety disorder (GAD), which is characterised by continued feelings of worry, fear and unease that are present for much of the time and not restricted to specific situations.
People with panic disorder often worry about when the next attack will happen and actively try to prevent future attacks by avoiding places, situations, or behaviors they associate with panic attacks. Worry about panic attacks, and the effort spent trying to avoid attacks, cause significant problems in various areas of the person’s life, including the development of agoraphobia (see below).
